We do remove the shackles and replace them before anesthesia wakes the patient up. The guard or guards usually sit just outside the OR door after the patient is asleep then comes back into the OR just before the patient brought out for under anesthesia.. I do not know of a case where the bovie burned the patient due to shackles, but I could happen let me know what you find out

While doing an EGD, a prisoner had arrived to the OR suite with shackles on his ankles and wrists. The wrist shackles were secured to a large leather belt around the patient’s abdomen. In order to obtain an optimal position for the procedure, one of the wrist shackles had to be removed. Two armed guards also accompanied the patient. Once the patient is asleep, should the shackles be removed, and the guards leave the room? Has there been a documented case of a bovie burn due to shackles?
